Hepatocellulært karsinom er vanskelig å behandle fordi de aller fleste tilfeller oppdages først når kreftsykdommen er utbredt, eller det foreligger betydelig nedsatt leverfunksjon. Men som ved andre kreftsykdommer, gir tidlig diagnose bedre muligheter for overlevelse. De mest aktuelle behandlingsmetodene er å fjerne den syke delen av leveren (leverreseksjon), eventuelt utføre levertransplantasjon eller lokalbehandling med f.eks. blokkering av blodtilførselen til svulsten eller stråler. Siden det er stor mangel på givere av lever, er levertransplantasjon sjelden aktuelt. Lokalbehandling har kun en lindrende effekt.
