Autismespekterforstyrrelser og kosthold

Kostholdets innvirkning ved autismespekterforstyrrelser har blitt undersøkt i en rekke studier. Kostomlegging i håp om å bedre tilstanden er et kontroversielt tema. Forskningen sier nei, men ikke alle ekspertene er enige.

Autismespekterforstyrrelser (autism spectrum disorder, ASD) er en samlebegnelse på ulike varianter av autisme. Autisme kan inndeles i ulike alvorlighetsgrader.

Barneautisme, Aspergers syndrom og uspesifisert gjennomgripende utviklingsforstyrrelse ble opprinnelig brukt om ulike grader og typer av autisme, men fagmiljøene har etterhvert gått over til å bruke autismespekterforstyrrelse om alle grader av autisme.

Det finnes noe data på at enkelte personer med autisme har ernæringsmessige mangler og avvik i forbrenning og omsetning av enkelte næringsstoffer. Det er imidlertid ikke klarlagt hvorvidt dette forårsaker eller påvirker autisme, og om det er et resultat av tilstanden eller er uavhengig av autismespekterforstyrrelsen. En lang rekke studier har blitt gjennomført for å se på effekten av tilskudd av vitaminer, mineraler og ulike diettbehandlinger ved autisme. Ser man på forskningsresultatene på dette området, er konklusjonen klar: det finnes ikke kvalitetsstudier som bekrefter sikker effekt av noen dietter eller kosttilskudd. 

Vitaminer og mineraler

Vitamin B er det kosttilskuddet som er grundigst studert hos personer med autisme. B vitaminer er egentlig en samling av vitaminer, som blant annet er nødvendige for utvikling og regulering av nervesystemet. Det er utført studier med tilskudd av vitamin B og magnesium, men det er fortsatt gjort for få studier, og de mangler tilstrekkelig kvalitet til at vi kan anbefale bruk av vitamin B6 + magnesium i behandlingen av autisme. Når det gjelder tilskudd med omega-3 fettsyrer, har studier vist motstridende resultater, og vi kan således ikke trekke sikker slutning om dette virker.

Kostomlegging

Ifølge norske eksperter anbefales ikke diettbehandling ved autisme på generell basis på grunn av mangel på gode vitenskapelige bevis for effekt. Vi vet imidlertid at et sunt og balansert kosthold er viktige forutsetninger for normal vekst og utvikling, og en god helse. En bekymring er at utelukkelse av enkelte matvarer i kosten hos barn, kan føre til et ubalansert kosthold og påvirke barnets helse negativt.

Det finnes beretninger om autistiske barn som er blitt behandlet med gluten- og melkefri diett, og som har vist store fremskritt. Gluten finnes i våre vanligste kornslag hvete, rug, bygg og havre. Kasein er et melkeprotein. Teoriene bak en slik effekt har gått på at disse proteinene brytes ufullstendig ned i tarmen, tas opp i sirkulasjonen og påvirker hjernen negativt hos noen av barna med autisme. 

Det er gjort noen få kontrollerte studier på effekten av gluten- og kaseinfri diett ved autisme, men studiene er av for lav kvalitet til at man kan konkludere, både når det gjelder effekt og skadeeffekt av slike tiltak. Ut ifra dette anbefales ikke glutenfri eller kaseinfri kost til barn med autismespekterforstyrrelser med mindre barnet har fått påvist cøliaki eller gluten hypersensitivitet. Andre dietter som er noe studert, er blant annet ketogen diett (fettrik, karbohydratfattig kost) og Feingolds diett (eliminasjon av enkelte tilsetningsstoffer i kosten).

Flere samleanalyser (metaanalyser) konkluderer med at det ikke finnes kvalitetsstudier som bekrefter noen effekt av ulike dietter eller kosttilskudd. Unngåelse av enkelte matvarer anbefales kun dersom det er påvist allergi eller intoleranse eller der barnet har mage-/tarmplager som kan behandles med endringer i kosten. Dersom du allikevel ønsker å prøve ut en alternativ kostplan for ditt barn, rådfør deg med barnets lege.
